OnBase Electronic Forms

Office of the Registrar has been working in partnership with the Office of Information Technology to convert our paper-based student services forms and processes to a fully electronic format.   As forms go live in an electronic format, a brief faculty how-to-guide will be added next to each form.  

Instructors will have the ability approve or decline electronic form in OnBase.  Decisions will be made with the click of button, communication with the student will be automated via email, and overrides (if needed) will be placed instantly in the student registration system.

Prompt attention to these requests is essential, since the status of students’ class registrations depends on the approvals granted by these forms. This is especially true as the add deadline approaches each term.

We will also be rolling out other electronic student services forms on a similar format and approval processes.

 


Late Course Add Form

Add deadlines
- Week 1: A student should not be using this form to obtain your signature during Week 1. If the student is attempting to obtain your signature for a prerequisite waiver, they should be using the Requisite Override Enrollment form. Students are not allowed to add into classes that are closed.

-Week 2-3: Students are required to obtain Instructor signature after the first week of instruction. A student may add using the Late Course Add Form only with Instructor's approval if all requisites have been satisfied and the course is not at capacity.

Drop deadlines
Instructor consent not necessary. Students may drop online without any further approval. Once the deadline has passed, students may withdraw using the Course Withdrawal form. OnBase Faculty Guide


Concurrent Enrollment Add

Concurrent Enrollment students will seek your approval on this form after attending the first class meeting. It is up to the instructor to determine if there will be space in the course after all matriculated students have enrolled. Concurrent Enrollment students may not take independent study or research courses. 


Course Withdrawal

Students may seek approval for a course withdrawal after the drop deadline and before the 11th week of instruction. A student may withdraw from your course provided 1) the student is not on special probation (contact your school's academic advising office), 2) dropping the course would be to the educational benefit of the student (specific school rules apply), and 3) the student is not being investigated for academic dishonesty in your course.  OnBase Faculty Guide


Credit by Exam

Determine if the student's knowledge of the subject may be tested by examination and if the student's preparation for the examination appears satisfactory. Certain courses may not be considered appropriate for obtaining credit by examination, such as courses with extensive laboratory work.


Incomplete Grade

An I grade may be assigned when the instructor determines that a student’s work is of passing quality and represents a significant portion of the requirements for a final grade, but is incomplete for a good cause. This form must be signed by both the student and Instructor and returned to the Students First Center by the first day of instruction the following term. 
 

Independent Study Enrollment

Students must complete this form if they you wish to enroll in Directed Group Study, Directed Independent Study, or Research.  The form will be routed automatically to the selected instructor and Dean for approval.  The student will be automatically notified when their request is finalized and either approved or denied.    OnBase Faculty Guide


Petition of Academic Policy

Students must submit a statement of support from each instructor for every course change being petitioned. Examples of statements to include: confirmation of attendance, the start and/or end dates of participation in the course, other applicable information to the situation.


Requisite Override Enrollment

Students may seek Instructor approval to add into a course in which they do not meet the prerequisites. The Instructor should use the information on this form to determine if the student will be able to succeed in the requested course without the prerequisites.   OnBase Faculty Guide


Time Conflict Override

It is the policy of UC Merced not to simultaneously enroll students in any course meetings or final examinations during the same time period without both instructors’ consent. Students must obtain each instructor’s signature. Students should also be advised that overriding a course time conflict could create a final examination time conflict.   OnBase Faculty Guide
